The NHS offers patients aged 40-74 an MOT to look at their risk of developing heart disease or stroke. This assessment requires you to have a fasting blood test and then a consultation to look through your personal and lifestyle risk factors. Please either phone the surgery to make an appointment for an NHS Check or complete this form below and our reception team will contact you. These checks are available every 5 years and are only for patients who do not already have heart disease, diabetes, kidney disease or a history of having had a stroke.
